Beyond Traditional Home Care


OPTIMAL CARE is a MEDICARE certified home health agency.  Reaching far beyond the traditional surgical and rehabilitation home care into specialized programs for seniors in their homes.

 Program specialities include:  geriatrics, behavioral health and dementia care.

 
Care Led by Advanced Clinicians


Our programs are developed and led by Advanced Practice Nurses (APN’s) who specialize in Geriatric and Adult Mental Health and Psychiatric Care. They have the expertise to provide advanced assessments, monitor and recommend medications adjustments and provide psychotherapy.
Geriatric Care


Needs change as we age. Our advanced level staff are equipped to address needs physically, emotionally, mentally and spiritually.  Clinicians are on hand and welcome even the most complex cases.

 
Behavioral Health & Dementia

A psychotic episode, a newly diagnosed anxiety or mood disorder or a long standing depression?  Our staff are trained to recognize these behaviors and implement effective therapeutic interventions immediately. We understand and treat dementia while supporting the patient and family through the challenges of the disease.
Patient & Family Education
